Students are required to submit weekly reflective narratives throughout the course that will culminate in a final, course-long reflective journal due in Topic 10. The narratives help students integrate leadership and inquiry into current practice.
This reflection journal also allows students to outline what they have discovered about their professional practice, personal strengths and weaknesses, and additional resources that could be introduced or recommended in a given situation to influence optimal outcomes. Each week students should also explain how they met a course competency or course objective(s).
In each week’s entry, students should reflect on the personal knowledge and skills gained throughout the course. Journal entries should address one or more of the areas stated below. In the Topic 10 graded submission, each of the areas below should be addressed as part of the summary submission.
1. Health disparities
2. Practices of culturally sensitive care

WK 1, I did New Practice Approaches
Professional Capstone and Practicum Reflective Journal week 1
New Practice Approaches
As nurses, we work in challenging and complex settings. Therefore, it is necessary to acquire both knowledge and practical experiences that will enable us to cope with the challenges and complexities of today’s health care systems in order to provide quality health care services to patients. My work experience dealing with patients in different healthcare settings and my learning and practical experiences here in this .. BSN program have improved and will continue to improve my ability to apply leadership skills and contribute to the delivery of effective healthcare services to the patients we serve.
The nursing profession has experienced major changes in the recent years. There is no doubt that having my capstone practicum done will afford me a significant opportunity to gain new knowledge and improve my skills as relates to providing up to date care to the patient. The BSN program has taught me how to adapt to the changes to remain up to date in my effort to provide efficient care to patients. Evidence-based practice has revolutionized the field of nursing and healthcare in several ways. For example, the use of evidence-based practice in nursing has prompted some adjustments in patient care in some health care problems like falls, diabetes, and obesity (Kristensen, Nymann & Konradsen, 2015 – Research Paper Writing Help Service). Unfortunately, these changes toward EBP in healthcare are not very much embraced in care delivery and various practices. The good news is that many higher institutions of learning are progressively recognizing the importance of this current trend by introducing changes to include evidence-based practice in their nursing programs. Today, the nursing profession has provided major leadership for improving care through the utilization of EBP.

WK 2, I did Interprofessional Collaboration
Week 2: Capstone and Practicum Reflective Journal
Interprofessional Collaboration
Interprofessional collaboration in health-care can be described as a situation where multiple healthcare professionals with different skills, specialty, and disciplines come together and work as a team to provide comprehensive and quality care to the patients, their families, caregivers, and the community (Green, & Johnson, 2015 – Research Paper Writing Help Service). In other words, we can call it teamwork. All through my practice as a nurse and also during the practicum, I have seen the importance of interprofessional collaboration in the provision of quality care to our patients. Health care practice is complex and requires more than one professional to deliver safe and efficient care. Often many errors arise from communication breakdown or ineffective communication. Such communication gaps can be filled when healthcare professionals work together; thus, reducing medical errors and increasing patient safety. When patient care needs are confronted from various angles and by different disciplines, it resulted in positive and outstanding patient outcomes (Bosch & Mansel, 2015 – Research Paper Writing Help Service). This is possible because interprofessional collaboration brings together different skills, talents, knowledge, and resources to handle the patient’s health care problem. For all the healthcare professionals to work together, they all must respect the expertise of each other’s disciplines. They must have common goals and communicate clearly and respectfully among the members of other disciplines.
During this practicum, I have worked collaboratively with healthcare professionals from other disciplines such as physicians, physical, speech, respiratory, and occupational therapists, social workers, dieticians, and pharmacists, and of course other nurses. All these professionals communicated with each other and share ideas concerning the patient’s condition and possible solution. One health condition may require the input of many other professionals from different specialties. Interprofessional collaboration benefits both healthcare professionals and the patient as no single person can perform all the healthcare needs of a patient alone. Interprofessional collaboration is also important for effectively solving of problems of improving medicine and technology, reduced resources, as well as the increased demand for healthcare. More than ever, the healthcare system needs a collaboration effort to patient care to promote health and prevent any preventable illness.
