This assignment is a mandatory submission, which means that in order to pass this

This assignment is an opportunity to demonstrate how you would provide person-centred care to
an individual, during one specific stage of the lifespan. You will select one case study to focus
your assignment on. By analysing and responding to this case, you will showcase your ability to
understand the needs of an individual, and your skills in developing a care plan based on those
specific needs.
By completing this assignment, you will develop your understanding of how a person-centred
care plan can be constructed from the information given to you by the person.
This assignment supports Subject Learning Outcomes 4, 5 and 6.

You are required to select one (1) of the following case studies as the focus for your
assignment:
• Option 1: Assignment 3: Case study 1 – Adolescence.

To select your case study, read through each case in detail and consider what you know about
the person’s stage in the lifespan. Select your case based on your level of knowledge, and your
personal interest in providing stage-specific care for the person described.
Case study analysis
Once you have selected and examined your case study, answer the following questions as they
relate to the case:
1. What is the person’s primary concern? How has this concern impacted their life, and their
relationships with family, friends and the community?
2. What is the person’s stage in the lifespan? According to Erikson’s psychosocial theory, what
challenge is the person facing? How the person is navigating this challenge?
3. Discuss the facilitators and barriers that the person may face as they navigate ways to
resolve their concern?

4. How will you, as a nurse or midwife, assist the person to find person-centred solutions to
their dilemma?
Note: Ensure that your answers are based on high-quality evidence found within current nursing
literature (published within the last five years). Look for peer-reviewed articles in reputable
journals and databases. CINAHL and Ovid Medline are useful places to start.
Case study presentation
Once you have answered the case study analysis questions, transform your work into an essaystyle response. Work presented in essay format should have an introduction, middle and
conclusion.
As a general guide, you may want to turn your responses to questions 1–3 into an introduction,
transform questions 4–6 into the middle section of your essay, and conclude with your thoughts
on question 7.
