Report major illness, surgery, injuries, or substance use/abuse here. Please include a history of any of the aforementioned. Additionally, if client reports no significant Health History indicated this by stating “No significant health history reported by client.”

vi. CLINICAL IMPRESSIONS:
This is where you can discuss your clinical impressions about client including prognosis, as well as any behavior that you feel is particularly important to note for continual observation. You conclude this section by including a diagnostic impression.

vii. DSM-V diagnostic impressions: This is where you write the Diagnostic Code consistent with DSM-V format & ICD 10 (i.e., 309.81 posttraumatic stress disorder, F43.1).
