Patient Case Study 1:
HH is a 68 yo M who has been admitted to the medical ward with community-acquired pneumonia for the past 3 days. His PMH is significant for COPD, HTN, hyperlipidemia, and diabetes. He remains on empiric antibiotics, which include ceftriaxone 1 g IV qday (day 3) and azithromycin 500 mg IV qday (day 3). Since admission, his clinical status has improved, with decreased oxygen requirements. He is not tolerating a diet at this time with complaints of nausea and vomiting.
Ht: 5’8” Wt: 89 kg
Allergies: Penicillin (rash)
Patient Health Needs
HH is a 68-year-old male with community-acquired pneumonia (CAP).
He has a history of COPD, hypertension, hyperlipidemia, and diabetes.
He is currently on empiric antibiotics, which include ceftriaxone and azithromycin.
His clinical status has improved since admission, but he is not tolerating a diet and is complaining of nausea and vomiting.
Treatment Regimen
I would continue the current antibiotic regimen for a total of 10 days.
I would also recommend adding a proton pump inhibitor (PPI) to help with the nausea and vomiting.
I would monitor his blood pressure, blood sugar, and lipid levels closely while he is on antibiotics.
I would also recommend that he get a flu shot and pneumonia vaccine to help prevent future infections.
Patient Education
I would educate HH about the importance of completing the entire course of antibiotics, even if he starts to feel better.
I would also explain the potential side effects of the antibiotics, such as nausea, vomiting, and diarrhea.
I would encourage him to drink plenty of fluids and to eat a healthy diet.
I would also recommend that he get plenty of rest.
Justification
The current antibiotic regimen is appropriate for the treatment of CAP.
